What it is
Berberine is a natural isoquinoline alkaloid extracted from the roots, rhizomes, and stem bark of medicinal plants such as Berberis aristata, Berberis vulgaris, and Coptis chinensis. In dietary supplements, it is standardly supplied as berberine hydrochloride (HCl), a crystalline yellow powder. It acts intracellularly as an activator of adenosine monophosphate-activated protein kinase (AMPK), a master metabolic regulator governing glucose uptake, lipid oxidation, and cellular energy homeostasis.

Berberine exhibits low oral bioavailability (~5%), meaning the majority remains within the gut intestinal lumen where it directly interacts with the gut microbiota. Commensal probiotic bacteria convert berberine into dihydroberberine via bacterial nitroreductase enzymes, a reduced form that displays up to five-fold higher intestinal absorption. Once absorbed across the enterocyte membrane, dihydroberberine oxidizes back into active berberine in intestinal tissue. Furthermore, probiotic short-chain fatty acids (SCFAs) act synergistically with berberine to strengthen tight junctions, lower mucosal inflammatory markers, and modulate gut peptide secretion (such as GLP-1) for enhanced metabolic regulation.

Safety & who should check first
Well tolerated at recommended doses; temporary mild GI cramping or constipation may occur. Not recommended during pregnancy or breastfeeding due to potential uterine stimulation and bilirubin displacement.
